VisionCare Plans for Individuals or Family

These VisionCare Plans are network-based vision plans that emphasize high quality routine eye care. Services and materials are provided on a pre-paid basis, and the plans pay network doctors directly.

VCP members have access to one of the largest vision networks in the United States, with more than 22,500 participating optometrist and ophthalmologists – and everyone accepts new patients. Plus, members save on frames. Members pay the wholesale price, avoiding high retail markups.

How do the plans work?

Members simply select any in-network optometrist or ophthalmologist and make their appointments. At the time of the appointment, members pay only their co-pays and for any extra cosmetic options selected. There are no forms to complete or claims to file.

Accident Plans for Individuals or Family

An Accident Compensation Plan pays cash benefits for accidental injury directly to you, the insured policyholder. Some of the main features of this plan are:

◆ Issue ages 0-63
◆ No reduction in benefits for change of occupation
◆ Full 24 hours coverage – on or off the job
◆ Air travel included
◆ Worldwide coverage

You can choose the "Cash Benefit" payout that is most beneficial to YOU as either:

  • Lump Sum - up to $10,000 per accident. The plan pays cash as specified for an accidental injury as listed in the Schedule of Benefits. If multiple injuries occur in one accident, this plan pays the highest incurred injury amount.

  • Hospital Stay Benefit - up to $6,250 per hospital stay. The plan pays $250 per week (pro-rated $35.71 per day) for as long as 25 weeks while confined in the hospital due to accidental injury.

FIRST AID BENEFITS — the plan pays incurred expenses up to $40 for medical treatment in the doctor's office or at the hospital when special outpatient treatment is required due to accidental injury and no other benefit is payable.

The plan also provides an Accidental Death Benefit that pays $10,000 upon an accidental injury resulting in the death of any covered person.

Disability Plans for Individuals or Family

These plans are available through your employer and include both Short-term disability and Long-term disability

Short-term disability (STD) insurance is a benefit for temporary disability due to sickness or injury. It pays a percentage of your salary for a short period of time.

How it works - Short-term disability reimburses up to 66 2/3% of the insured’s weekly earnings. In addition, your plan may include services to help you return to work after an illness or injury.

Long-term disability (LTD) insurance kicks in once your short-term disability benefits run out. The policy reimburses up to 66 2/3% of your income. The percentage and length of payment vary by employer. In addition, your employer’s LTD plan may include return-to-work support and rehabilitative services. These services are designed to help you realize the goal of recovery and a return to independent, productive work.

When you have a group long-term disability plan, your employer may pay part of the premium.
